You’ll receive on-call hospice support 24 hours a day, 7 days a week. Having a nurse on call gives you peace of mind and a trusted resource that can relieve stress. Family members especially appreciate our 24/7 support from our care team.
PHYSICIAN-DIRECTED CARE
Our licensed physician assists the hospice team with establishing and supervising your personal plan of care.
MEDICAL EQUIPMENT & SUPPLIES
Medical equipment & supplies related to the patient’s hospice diagnosis will be delivered to their place of residence by a reliable medical supply company. This equipment can include, but is not limited to:

Hospital bed
-
Wheelchair
-
Walkers
-
Lifts
-
Oxygen equipment
Medical supplies related to the patient’s hospice diagnosis are also provided at no cost. These supplies may include, but are not limited to:

Wound dressing
-
Incontinence supplies (bedpans, under-pads, adult briefs)

Gloves
EMOTIONAL & SPIRITUAL COMFORT
One of the greatest benefits of hospice care for the caregiver is the emotional support and education provided by our team. Our trained and experienced chaplains also provide spiritual support to patients and loved ones.
NURSING VISITS
Our nurses administer your medical and physical needs, and help to coordinate all components of your treatment plan.
MEDICAL SOCIAL SERVICES
Our Medical Social Workers help patients overcome some of the things that can get in the way of recovering from an illness or injury. Our Medical Social Workers perform home assessments to identify social, emotional, and financial concerns of the family & patient. Our Medical Social services are inclusive of, but not limited to the following:

DIETARY COUNSELING
Our experienced Dieticians assess the nutritional needs of our patients and provide individualized dietary programs and training to help support the patient’s overall health and recovery through personal nutritional programs. Our Dieticians are experts in the proper preparation, intake and choice of foods. Our Diet & Nutritional services are inclusive of, but not limited to the following:

Assess the patient’s nutritional needs

Evaluate and report the results with doctors and other health care professionals to coordinate medical and nutritional needs
